As the developers of the game had abundantly anticipated, in The Witcher 3 there are several epilogues of the main story and what each player gets will be strongly influenced by the choices made during the "wild hunt".

The minor variations and nuances could be many more, but the main endings seem to be essentially 3, with the third having more consistent variations.

ATTENTION!!! SPOILERS FOLLOW

First Final
Make 3 of the choices below to finish with the mission "Lament for a Dead Swallow" and get this ending:

• Act 2 - In the main mission "Blood on the battlefield" you choose to drink with Ciri and avoid the snowball fight.
• Act 2 - In the main mission "Blood on the Battlefield" you choose to take compensation from Emhyr when you return to him with Ciri.
• Act 3 - In the main mission "Final Preparations" talk to the Lodge of the Sorceresses instead of letting Ciri go alone.
• Act 3 - In the main mission "Preparing for Battle" you choose to give Lara Dorren's necklace to Ciri instead of having Avallac'h's laboratory ransacked.
• Act 3 - in the main mission "Preparing for Battle" DO NOT let Ciri visit Skjall's tomb in Hindarsfjall.

Second Final
Make at least 3 of the choices below to finish with the mission "The Impatient Slayer" and get this ending:

• Act 2 - main mission "Blood on the battlefield", it is chosen to engage in a snowball fight instead of drinking with Ciri.
• Act 2 - main mission "Blood on the battlefield", you have chosen to refuse compensation from Emhyr when you return to him with Ciri.
• Act 3 - main mission "Final preparations", you have decided to make Ciri speak alone with the Lodge of the Sorceresses.
• Act 3 - main mission "Preparations for battle", you allowed Ciri to loot Avallac'h's laboratory instead of giving him the necklace.
• Act 3 - main mission "Preparations for the battle", you have found time to visit Skjall's tomb in Hindarsfjall with Ciri.

Furthermore, it is also necessary to have performed these actions:

• Act 2 - In the main mission "Blood on the Battlefield" you have to agree to visit Vizima and introduce Ciri to Emhyr.
• Act 2 - Make sure you have completed the assassin missions (side quests): An Eye for an Eye, A Deadly Plot, Rediana the Wanted.
• Act 3 - In the secondary mission "Reasons of State" you have to: kill King Radovid and side with Roche.

Third Final
To get this ending you need to make 3 of the following choices:

• Act 2 - main mission "Blood on the battlefield", it is chosen to engage in a snowball fight instead of drinking with Ciri.
• Act 3 - main mission "Final preparations", you have decided to make Ciri speak alone with the Lodge of the Sorceresses.
• Act 3 - main mission "Preparations for battle", you allowed Ciri to loot Avallac'h's laboratory instead of giving him the necklace.
• Act 3 - main mission "Preparations for the battle", you have found time to visit Skjall's tomb in Hindarsfjall with Ciri.

But in addition to this you will also need to:

• Act 2 - main mission "Blood on the battlefield", make sure you don't visit Vizima and therefore don't introduce Ciri to Emhyr.

• Act 3 - In the secondary mission "Reasons of State" you have to: kill King Radovid and side with Dijkstra against Vernon Roche and Thaler.

Third variation of the third final

• Act 2 - You must have completed the assassin missions (side quests): "An Eye for an Eye", "A Deadly Plot" and "Redania the Wanted".
• Act 3 - DO NOT do the secondary mission "Reasons of State"
