Taylor should be the first survivor you meet shortly after leaving the diner and getting a truck early in the game.

EVERYTHING you will be able to collect will be considered a weapon (even a hairdryer), so any object that comes within range, take it; it is not necessary to equip it in the inventory, but simply pick it up and hold it in your hand, then you can throw it where you found it.
Get to 250 weapons and you will unlock the achievement.

Loot every piece of clothing you encounter during the course of the game, from shoes, to hats, to fun costumes.
There are tons of clothing stores scattered throughout Los Perdidos, and all you need to do is put on the garment.
Once you have it up, save it safely inside the closet in your home and later (if the new garment doesn't suit you) you can put on the old one again.
